State Fee is $55.50 ($37 each additional copy) which includes overnight shipping once issued.

Additional Requirements: Copy of state issued ID or Drivers license when faxing your order. If you process your order with our online process you do not need to send a copy of your drivers license or state issued ID. If your parents were not married at the time of your birth you must send a copy of your drivers license (this applys if you use the online or fax form).

If you use the online order form to order you do not need to fax a copy of your drivers license or anything else to complete your order. Everything is entered online.
