Additional Requirements: The state of Nevada may require one of the following: Photo Drivers License, Photo ID Card, US Passport, Military ID Card or Two of the following: Work ID/ Paycheck stub (cannot be handwritten), Voter registration card, Social Security card, or Motor Vehicle registration/title. We cannot obtain the birth certificate if the person was adopted or getting the birth certificate for an adoption.
