Additional Requirements: The state may require a copy of photo I.D. Which can be either be a state issued drivers license, state issued ID card, copy of a US passport, Military ID or Tribal ID card. If you do not have one of the previous forms of ID you can send 2 of the following with your current address: Utility bill, Social Security card, work ID card, valid motor vehicle registration (no titles), insurance record, library card, Rental/Lease agreement (must be 6 months old), current voter's registration or previous year's signed tax return (no W-2's).
