Additional Requirements: Must fax copy of photo I.D. Which can be either be a state issued drivers license, state issued ID card, copy of a US passport, Military ID, valid school ID, Veterans ID or Probations/Corrections ID. If you do not have one of the previous forms of ID you can send 2 of the following with your current address: Social Security card, valid Indiana Gun Permit, valid motor vehicle registration (no titles), Rental/Lease agreement (must be 6 months old), military DD-214, current voter's registration or previous year's signed tax return (no W-2's).

Indiana state law requires someone sign for the birth certificate when it is delivered by an overnight courier service. You cannot leave a signed letter on your door asking the overnight courier to leave the package.
