Additional Requirements: Valid photo ID or 2 Work ID/paycheck stubs (not handwritten), doctors medical record, canceled check (not voided), Social Security card or court order of adoption /legal name change. The shipping address of the birth certificate has to match the credit card billing address (this is usually your home address). This is a state requirement.
