Additional requirements: We must have a copy of the applicants' or at least one parents' valid state issued drivers license or ID card. The address on this ID must match the shipping address on the following form. If they do not match you will need to provide at least 2 of the following that have the same address as the shipping address on our form:

utility bills, credit card bills, bank statements (you may mark out the account numbers) or mortgage payments.

If your name is different from your birth name because of marriage you MUST provide a copy of your marriage license. This is a New Jersey state requirement.

We cannot obtain your birth certificate from New Jersey if you were adopted or born in a military hospital.

We cannot process birth certificates from Hudson county or Atlantic city.

The Colorado Department of Motor Vehicles will not accept the birth certificate we obtain for you to obtain a new/renewal of a drivers license, they require a special copy that we cannot obtain for you.

You will send your order to us by fax, we need a signature. We will process your request and have your original certified birth certificate with a raised seal and/or a water mark sent to you by an overnight courier service(Fedex, UPS etc.).
