The ship to address name must be the same as the certificate holders name. Someone must be at the delivery address to sign for the overnight package, no exceptions. The ship to address must be the same as where your credit card billing statement is mailed (this is usually your home address).
Credit Card must be in certificate holders name, if the request is for a child the credit card will need to be one of the parents.
If you use the online order form to order you do not need to fax a copy of your drivers license or anything else to complete your order. Everything is entered online.
