Additional Requirements: We must have a copy of the birth certificate holders drivers license, state issued identification card, school/college ID, Passport, Tribal ID card or Naturalization certificate. If the applicant is a minor then we will need a copy of one of the parents drivers license or state issued identification card. If you do not have one of the previous you will need to provide at least 2 of the following: work ID card, paycheck stub, W-2, voter registration card, Social Security card, DD 214, motor vehicle registration or title, probation documents, marriage license, Medicaid card, Social Services card, Department of Corrections ID card.

The birth certificate can only be shipped to your credit card billing address (where you get your bill every month).
