Additional Requirements: We must have a copy of the birth certificate holders Drivers License (or parent's if for a child) with your current shipping address. The birth certificate must be shipped to the address on your Drivers License which must also be your credit card billing address. If your Drivers License address is not the shipping address you must provide 2 proofs of residency which can be a utility bill, credit card bill, W2, pay stub, bank statement or car registration. If your name has changed from your birth certificate because of marriage you will need to provide a copy of your marriage certificate. Please upload copies of these on the order form.

New Jersey birth certificates cannot be shipped internationally and cannot be shipped by regular mail. Sorry, all the above are state requirements.
