Additional Requirements for online orders: Shipping address must match the credit card billing address on the state issued ID card or Drivers License. The credit card MUST be in the certificate holder's name, a parent, grandparent, spouse or adult child.

If you do not have a Drivers License or state issued ID card, or the address on the ID card or Drivers License doesn't match the shipping address you must provide 2 of the following:

Utility bills (gas, water or power dated within the last 90 days), car registration, pay stub, bank statement, Income tax return or W-2 or lease/rental agreement.
