Additional Requirements for online orders:

If you use the online order form to order you do not need to fax a copy of your drivers license or anything else to complete your order. Everything is entered online.

We must have a copy of the credit card holders state issued drivers license or ID card if the order is faxed. If the drivers license address does not match the shipping address or credit card billing address you must fax a copy of 2 utility bills to prove your delivery address. Must fax a valid state issued photo id (if you don't have a valid ID please see above for details). V- code required for credit card.
