Additional Requirements: Must fax copy of one of the following: state issued photo ID, photo ID card, US passport or US military ID card.

We MUST have the birth certificate holder or a parent's (if the parent is ordering) Social Security number on the order form. The state of Texas requires this.

If you do not have one of the previous then 2 of the following will be needed: social security card, utility bills with currect address, school/college ID card, letter from employer on company letterhead (must have phone number for verification purposes) or bank statement with current address.
